New Student Enrollment

The following documents are required to complete the enrollment process for new students and should be presented at the time of registration:

1.  Proof of Guardianship: Birth Certificate with parent name on it

    or Foster Care Documentation

    or Legal Guardianship Documentation

2.  Proof of Residency: Lease or Contract on a home

3.  Two utility bills in parent name

4.  Immunization Record

5.  Birth Certificate

6.  Social Security Card

7.  Last Report Card

8.  Recent Standardized Test Scores
